Discharge
Planning for Hospitals and Long-Term Care Facilities
Situation:
In the mid to late
1990’s hospitals faced significant expense from delays from
patients who could not be efficiently discharged to outpatient
and long-term care facilities.
Challenge:
Hospitals were
dependent on the time consuming process of local
representatives from area facilities visiting discharge
planning offices and reviewing clinical information. Families
could not easily evaluate various outpatient care options
which further delayed hospital discharges. The widespread
introduction of Internet access provided new opportunities to
streamline patient flow.
Action:
After an analysis of hospital discharge planning needs,
CareView, a multimedia information guide to Skilled Nursing
and Assisted Living Providers, was created in 1998 in order to
provide a more efficient discharge planning system. With
CareView's comprehensive database and complete listings, Case
Managers, patients and family members can preview many
facilities with the click of a mouse. Narrated video
"tours" and detailed descriptions enable those
involved to make informed decisions regarding a facility,
based on their individual needs. CareView helps hospitals
locate and communicate discharge planning information with
long-term care facilities, by enabling case managers to search
for the most appropriate long-term care facility that matches
patient and family criteria, and send discharge planning
information in electronic form to eligible homes.
CareView helps hospitals increase their patient
throughput, reduce length of stay, maximize asset utilization,
and eliminate workflow bottlenecks by eliminating the tedious
and time-consuming tasks associated with matching patients
with providers.
Results: Created and installed CareView at 350 Hospitals with a database of more than 2,800 professional health care users, which created significant opportunities for reductions in lengths of stay in the millions of dollars. Customers include Columbia Presbyterian, Jackson Memorial, and New York Hospital. In 2001, coordinated the integration of CareView with the Curaspan Edischarge Platform in order to facilitate further enhancements.
